Who can access our services?

To access the Home from Home or One to One Service:

• You need to contact Social Work on 01324 506070, who will complete an assessment and then refer you direct to JDI if we meet your needs.
• You need to be a resident within the Falkirk Health and Social Care Partnership area.
• You need to have a diagnosis of dementia.

To access our Groups:

• You should complete the referral form below and have this signed by either your GP, nurse, social worker etc.
• If you are a resident within the Falkirk Health and Social Care Partnership area.
• And have a diagnosis of dementia.
