The Joint Dementia Initiative is part of Falkirk Health and Social Care Partnership, the local body responsible for the planning and delivery of community health and social care services. We are supported by Falkirk Council’s Social Work team and NHS Forth Valley’s Dementia Services to meet the needs of people living with dementia in Falkirk.
Our services help people who have dementia stay at home and live within their communities for as long as possible. If you, or someone you care for, has dementia, we can help you:
- Find ways of keeping safe at home
- Identify familiar routines and patterns for everyday living
- Maintain skills, such as taking a bath, dressing, cooking or shopping
- Learn new skills, such as computer skills to help you keep in touch or new hobbies and interests
- Maintain friendships and relationships
- Be involved with your local community
- Get access to other services, such as health services
- With advice on specific issues that are happening in your life
- Support is different for everybody and is based on what you need
How do I access support?
Before you can receive support from the Joint Dementia Initiative, you need to have had an assessment by Social Work services. This means that a person from Social Work would contact you and have a chat with you about your needs. Contact Adult Social Work Services on 01324 506070.
Everyone who has been diagnosed with dementia has unique interests, personalities, backgrounds, values, health and emotional needs.
The Scottish Government has guaranteed a year’s post-diagnostic support for everyone diagnosed with dementia from April 2013. In many areas this is provided by Alzheimer Scotland’s Dementia Link Workers on behalf of the NHS. In other areas post-diagnostic support is provided by NHS staff.
